Slide from Nowhere?

I'm having trouble understanding how to play something along the lines of "/11" for example (I'm assuming this is a slide from nowhere).

Do I:

1) Hold down the appropriate string at a fret somewhere randomly below the 11th fret, pick the string (while still below the 11th fret) and slide up to the 11th fret in one action, or

2) Hold down the appropriate string at a fret somewhere randomly below the 11th fret, slide up to the 11th fret (without picking the string), and then pick the note when I get to the 11th fret.

3) Something else.

#1 seems to sound the best to me.

#1 will do nicely.

How far away will depend on the effect you what to achieve and how much time you have to do it.

I started with nothing - and I've still got most of it left.

#1 yes yes.
where you start creates a feeling.
what is common is two frets below and slide up.
another is mimicking a typical bend.
there shouldn't be hard rules.
what suits the song and your mood works best.
