Wow, this is clear and concise, but I have to add my 2 cents worth. If you do this right, you don't even have to more more than 1 finger at a time!

Here's how I do it:
Start with you third finger on the 3rd fret of the 5th string, your second finger on the second fret of the fourth string, and your first finger on the second fret of the third string.
Play the A chord with the 2nd, 3rd, and 4th strings as shown.
Move only your index finger to the 2nd fret of the first string, and play the D/F#. Your first finger will mute the 2nd string. Leave your 1st finger there.
Move your third finger to the third fret of the first string. Play the G5 chord there, your finger will automatically mute the 2nd string.
Go back and forth between those last two chords as shown. You'll end up on the D/F#.
Now just move your first finger back to the 2nd fret of the 3rd string. You're back where you started from on the A chord!
Now just practice it real slow, using your palm to mute a bit between chords. You'll soon find how easy and accurate this sounds!
