Bit difficult to explain this one, but......When TAB shows two strings (say 1st & 3rd) being played at the same time, whilst the string between has no indication of being played at all (not even muted), do they have to be played with fingers or can they be flat-picked?

Tab ain't your master, play it both ways and decide which way you prefer. You can just mute the B-string and hit all three of em, fingerpick or hybrid-pick it. It will sound different but which is best depends on your ears.
