I've seen lots of handy charts and stuff people have made for scales, the fretboard etc.
Has anyone put one together for common transitions with open chords?

Why use a chart when you can think for yourself?

More importantly, you can conceivably walk from any one chord to another. That's just a matter of the chords involved, the timing of the change and your imagination. A chart might be handy to get you started, but if you just get into the mindset that any chord change can incorporate a walk, then you won't need the chart at all.
