Is it best to use your thumb on the low E ?
Or do I make a D7 with my middle, ring and little and reach the index finger over to the F# ?

Either way. I think Eric uses his thumb on a lot of those types of chords.

I always play the D chord with an F# bass note....A) it's the way I was shown, B) F# - the note - is an essential component of the D chord and C) it gives you a fuller sound....
As to how to play it, I always use my thumb....find it easier to use a mini-barre at the second fret with my 1st finger, 2nd finger on the B string, 3rd fret...

Although the classical player in me is loathe to commit such an offense against "proper" technique, I think the thumb is pretty crucial. It allows you to switch to the D/F# from all the other open chords with basically the same motion, and it also puts the ring finger on the high E, which makes hammering on a lot easier.

Since this song is just straight strumming it really doesn't make a difference. Eric uses this same chord a lot in other blues songs where he does a walking bass line on the low E string (F#, F, F#), in which case he ends up with the middle finger on the F#, making it easier to play the bass line.
If you look at Eric Clapton in the DVD "Sessions with Robert J." in the tune "From four until late" hi is using the thum for F# on E6 string (in D7) but does a fast change to a new grip with index finger on F# on E6 string. I cant understand why, but hi excels in fast chord changes in that tune.
