I think Johnny Cash's version of "Hurt" is pretty powerful, especially when coupled with the imagery in the video.
Also, there's a pretty cool cover of the Tears for Fears song "Mad World" in the movie "Donnie Darko", although I don't know who's doing it.

The Johnny Cash version of Hurt is extremely powerful and emotional. When Trent Reznor saw the video for the first time he wept, apparently he never thought of the song that way when he wrote it.
Cash does a great version of Rusty Cage (original by Soundgarden) as well. He last album (American IV), was almost all cover tunes, fantastic album.
A couple of other tunes that was greatly covered is:

In most cases, I am not a big fan of cover tunes, but sometimes an artist can bring something new to song. It's rare in my opinion, but it can happen.
